ln
line with "minimally
invasive
surgery" principles, a classical
radio-
therapy
technique has been
revived
because
of
recent image
and
radioac-
tive seed availability
developments.
Radioactive
iodine-1
25 seed
implants
have been very popular
in
the
USA
be-
cause of its labour-saving, low-cost op-
eration and
the
very
short
stay in hos-
pital
required. Moreover,
in
expert
hands,
its
effectiveness
is
equivalent to
surgery. ln
fact,
in such experienced
hands, the potential to achieve lower
complication rates
is
significantly
greater with the implant method than
with
surgery.
Before
the implant method
was available, adverse effects of wide-
field conventional radiotherapy include
diarrhoea, rectal bleeding
(due
to radia-
tion
damage to
the
rectum), tenesmus
(sensation
of urge
to
open
the
bowels
despite
an
empty rectum), dysuria
(painful bladder sensation, especially
upon passing urine)
and
urinary fre-
quency/urgency,
etc.
Others
may even
get impotence or urinary incontinence,
due
to more
severe
tissue damage from
surgery
or
radiation. Thus,
this
tech-
nique
is
now
getting
more
and
more
popular
in
the USA
(however, in
other
much more
densely-populated
countries, as problems of cremation of
bodies still harbouring radioactive
im-
plants releasing
radioactive gases still
exist, the technique may be discour-
aged
by health
authorities
on safety
reasons
rather
than
medical reasons).
ln
these
regions, equally conformal but
safer
methods,
e.9.,
stereotactic
radiotherapy,
may be
more appropriate,
as decided entirely on individual condi-
tions and considerations.
