21st
Century, with

rap-
idly
ageing
population
in
many
countries, there would also be a sharp
rise
in
the incidence rate of
diabetes.
ln
the elderly,
40"/o
have
diabetes
and
in
those >65
year,
50%
have
type
ll
diabetes.
The greatest
problem of dia-
betes
is
its complications as
up
to 40%
of
diabetics
have such
complications
-
the
death
rate of those
with
vascular
episodes
referable
to the heart or the
brain is double that of the same
age.
The
UK Prospective Diabetes Study
(UKPDS)
is
the largest
type
ll diabetes
study.
During the
period
1977-91
,5102
cases
of
type
ll
diabetics were
recruited.
Results showed
that in
overweight
subjects,
an
anti-diabetic agent
called
metformin lowered blood
sugar
level
more effectively,
the
blood
total
cholesterol,
low
density
lipo-protein,
triglycerides were also lower than other
treatments; moreover,
the
incidence of
coronaries
and
angina
as well as
deaths
due to heart and blood vessel diseases
were also lower; metformin also sup-
pressed
blood clot
formation.
ln short,
metformin
not
only lowered blood sugar
well
but also other risk
factors
associated. Thus, for
those
cases who
lailed to
reduce
weight,
metformin ap-
pears
to
be the choice therapy.
