RENAL
HYPERTENSION
To institute the proper therapy
for
the patients with hypertension,
it
first
must be
determined whelher hypertension
is
due to
a
clearly definable cause,
i.e.,
secondary
hypertension such as
renal
disease; if
no cause
can
be identified,
essential (primary) hypertension
is
present.
Two subdivisions
ol
renal hyper-
tension, i.e.,
renal
arterial hypertension
and
renal parenchymal hypertension
have been
recognized.
Patients with
untreated secondary hypertension have
lactors
in
addition
to
the blood pressure
elevation which influence prognosis. An
example of
this
is
the
patients
with
hypertension due to renal parenchymal
disease, whose prognosis
depends
also
upon
the
nature
of
renal disease.
The effectiveness of surgical therapy
in
improving the prognosis
in patients
with
renovascular
hypertension
is
also
established, except in elderly patient
with
renal
artery
stenosis due
to
arteriosclerosis.
