USA
started
interventional
coronary treatments
in
the
70s. ln China, these
first began in
1980 and became
widely
avail-
able by
1988. Ever
since the early
90s,
when the Cardiology Department of the Shanghai
No.
6
People's
Hospital attached to the
Jiaotong
University embarked on cardiac interventional
work,
it has been very well
received.
They provide percutaneous transluminal coronary angioplasty (PTCA)
to
re-establish
the
lumen of
the coronaries
by
catheter and balloon, or stent placement; some patients require lasers
to "burn"
the occlud-
ing
plaques,
or
very
high
speed drills to re-create
a
passage. PTCA
is
a
good alternative
to
surgical operation,
coronary artery bypass grafting
(CABG).
PTCA is popular
in
China as
it is
significantly
safer and
cheaper
than CABG.
However, Prof Wei
was very skeptical on transmyocardial revascularization
(TMR):
finely
fo-
cused lasers perforating ischemic heart muscles
-
as
only
4o/o
of
all
his patients could benefit (as
PTCA
was
inapplicable).
As
restenosis after stenting may
at
times occur as early as 6 months, medication and lifestyle
changes must
never
be
ignored. Prof Wei
also reported on
effective prevention
of restenosis
by local drug
delivery systems to suppress "neointimal proliferation". Transfection
of
genes encoding angiogenic code for
biological
repair
at
the
disease
site
is in
the
pipeline.
