THE PAST
ANDTHE
FUTURE
OF HEARTTRANSPLANTS
As the misconception
that
heart
transplants would also "import"
the
mentality
of
the donor to the recipient
seems
still
prevalent, discussions
on
this
novel
surgical
method
are
still
required.
lf
the
heart
fails to pump blood
efiectively around the body, one way is
to
replace it
with
another compatible
and healthy human heart. The very
first
successful
human-to-human heart
transplant by Dr Bernard of
South
Al-
rica
in
1967 was remarkable,
even
though the patient died
18
days later
of
lung infection. ln
reality,
there are
still
many problems, e.9., ensuring very
promptly the donor heart to be not only
healthy
but
also
has
compatible
blood
grouping
and
HLA typing etc; the short
time
span
from taking
out
the
heart
from
the
donor
to
the
transplantation;
the
greatest hurdle
was
recipient rejection;
even post-operative inlection
affected
seriously success rates - related to
the
use
of
steroids
for
immunity
suppression. After the discovery that
Cyclosporin
A
could replace steroids
for
immunity suppression, such transplants
had a significant boost, as
infection
rates due
to
steroids could now
be
reduced. Nevertheless, issues of reluc-
tance to donate and the controversial
definition of "brain death" still exists but
"xeno-transplant",
e.9.,
from the
pig
or
artificial
hearts may
be
good
alternatives.
