“Minimal” Surgery for “Maximal Exposure” of the Base of Skull
Mdm Wang had a 6-cm trigeminal neurilemoma below her brain, on the Meckel’s cave in her cranial fossa, infiltrating the petrous
bone of the base of skull, compressing the brain stem and close to the cavernous sinus / carotid artery. Dr Zhang XiaoBiao, Head
Neurosurgeon of the Zhong Shan Hospital attached to the Fudan University, had the novel endoscopic transnasal approach through the
nostrils achieving a panoramic view enabling complete resections. She was up and about the next day! Dr Zhang opined the traditional
craniotomy approach required large, deforming scalp incisions; even for those much less extensive ones, the classical transnasal
approach employed a microscope with a narrow visibility. But, the novel approach has microinstruments for minimally invasive surgery
to resect large benign intra-cranial tumors, e.g., large infiltrating pituitary tumors, and others that infiltrate towards the suprasellar
region (above the pituitary), the third ventricle of the brain, e.g., craniopharyngiomas, sellar meningiomas, and even odontoid
resections, etc. – all may now use the novel approach.
