Blood Pressure Lowering in Midsummer: the Knowhow
Some hypertensives tend to stop their drugs in the summer as there is a natural tendency for the blood
pressure (BP) to drop in summer. Yet, it could just be a BP drop in the day time, but, at night, the BP
rises: do not stop the drug(s) but do keep on using them. In 2012, an Italian Hypertension Center reported
on 1395 patients' BP in summer months having lower daytime BP and 24-hour diastolic BP, except for
an inverse relationship on night-time BP and an isolated nocturnal high BP: higher or more common
in summer: likely due to improper drug reduction in summer. The European Society of Cardiology also
stressed (2013): nocturnal high BP is a strong predictor of cardiovascular prognosis. Actually, taking
antihypertensives involve “type”, “dose” and “timing”. Calcium-channel blockers and diuretics have better
systolic BP control; alpha, beta blockers, diastolic BP control. Never stop all the drugs in one go: do it
gradually. In summer, with drenching sweat, or, especially with associated diarrhea and vomiting, use
diuretics with caution. Norvasc, though long-acting, may best be taken at 8-11 pm if the BP tends to rise
later in the night, as its action starts to vane after 20 hours. In summer, beware of nocturnal high BP!
