PAIN FREE
There are 300-350 million cancer patients across the
world,
many
of them are
in
China.
Unfortunately,
50-80% of these
have
various degrees of
pain.
Any sub-optimal pain management would
thus
lead
to
unnecessary suffering
in
a huge
number
of
patients,
with
marked effects on
the
society.
Even
though heat
or
cold
compresses, massage, psychotherapy, physiotherapy
or
"counter-
irritants" etc.
may
relieve
milder
pain, moderate
to
severe
pain
re-
quires medication.TheWorld
Health
Organization
is
very
concerned
and has also devised the "WHO
Ladder" for
cancer pain control.
For mild pain, common
pain
killers;for
moderate pain, stronger
pain killers are
used;for
severe pain, morphine
is
advocated
-
alltaken
by
mouth and preferably "round the
clock", even
when
pain seems
to
be controlled temporarily. When
titrated
individually and precisely against
the severity of pain, addition would hardly
occur.
The most significant side effect of morphine
is
constipation.
The severity of pain should
also
be
charted
systematically
-
just
like blood pressure and pulse. No patient
should ever be allowed
to
"bear
the
pain". ln any case,
when
pain
is
well established, much higher doses of
pain killers required may have more side effects. Communication between
the
medical professionals and the
patient
or
relatives is
vital
not only
to
maximize management effectiveness but also
to
establish faith. Good
pain
control
means preservation of dignity
and
better quality of
life.
