OPTIMAL ANTITHROMBOTIC
THERAPY
FOR
CARDIAC
PATIENTS
For
cardiac patients, prevention of
blockage of
blood
vessels; reduce blood
clotting within blood vessels and
to
dissolve established blood clots
are
required.
All these are
collectively
known as "antithrombotic therapies".
There
are 3
main kinds
of
antithrombotics:
anticoagulants (blood thinning agents),
e.9., warfarin, heparin and
the
low-
molecular weight heparins; platelet
inhibitors, e.9., aspirin; and
thrombolytics
(clot dissolving
agents).
The
most
appropriate agents are chosen at
a
highly specific dosage
that
is
re-
adjusted regularly according to current
blood test
results.
There
is
also
a
whole
list of other drugs that would
cross-
react with these antithrombotic agents.
As under-dosage
leads to
failure to pre-
vent
blood
clotting and
over-dosage
leads
to
highly risky
bleeding,
like
strokes leading
to
gross
disability
or
bleeding into the gut
/
urinary tract or
f
rom
the
womb
(as very
profuse
"menses"), etc.,
dosage
determination
is
very meticulous and depends on the
expertise
of
the
doctor. Any self-medi-
cation or
dosage
self-adlustment
or
defaults would definitely be disastrous.
Normally,
the
medical
professionals
would
very
much prefer
to
check the
blood count,
PT,
APTT and
INR
(occasionally,
the
FDP and fibrinogen)
etc.,
forthe
degree
of blood thinning
way
before any detectable bleeding occurs.
By
that time, it
may
well
be
too
late
to
save
the
life if
bleeding
happens
to
be
profuse.
