INTRODUCTION TO
THYROTOXIC HEART
DISEASE
Thyrotoxicosis, due to excessive
secretion
of
thyroid hormone,
is
fairly
common.
Typically, the patient
has
heat intolerance, excessive
sweating,
weight
loss despite good appetite,
hyperactivity,
bad
temper,
palpitations
(excessive
heart
beat)
and
exophthalmos (prominent eye balls).
The
blood T3 and T4 levels are
elevated
and
TSH decreased. Thyrotoxic
heart
disease patients
have, in
addition, heart
problems due to the prolonged etfect
of
excessive
thryoid hormone.
After
excluding heart problems due to
other
causes, anti-thyroid treatment directed
to
reduce
the
excessive
hormone
secretion would be
very
effective tor
thyrotoxic
heart
disease.
Even before
hormone secretion
has been
optimized,
beta-blocker
drugs
like
propanolol
are
commonly used
for
symptomatiC
control.
Control
of
thyrotoxicosis
can
take
up
to 2-3 years, during which time
the patient shoulrd have adequate rest
and
nutrition and
to
avoid excessive
physical
activities.
The most important
point is to
comply
with the
recom-
mended
treatment,
otherwise
thyrotoxicosis would
be
ditficult
to
be
controlled and thyrotoxic heart disease
may be
complicated
by
conditions
like
heart
failure, coronary
insufficiency,
cardiac arrest (cessation of heart beat)
and even death.
