MISTAKES AND STRATEGIES
IN
TREATMENT
OF
HYPERTENSION
The aim of drug therapy
for
hy-
pertension
is
to
use
the
agents
(either
alone or in combination) to return arte-
rial
pressure to normal levels with mini-
mal side
effects.
These agents should
have dilferent site
ol
action.
Since
many effective antihypertensive agents
are available,
a
number of
useful
thera-
peutic regimens have been developed.
A plan of therapy should be suggested
for a patient
with essential hypertension
without contraindications to
specific
agents.
ln
general, there
are only
a
few
patients with whom blood pressure can
not be adequately controlled with the
above
approach.
However,
it
must be
emphasized that only
a
very small frac-
tion
(in
the United States, probably less
than
one{ifth)
of hypertensive patients
is
being treated
effectively.
lt
has been
demonstrated that only
a
small number
of
these failures
is related
to drug
un-
responsiveness. The majority are
re-
lated
to: 'l)
faiure of detection of hyper-
tension at early stage; 2) failure of
in-
stitution
ol
effective treatment of the
asymptomatic hyertensive subjects;
3)
lailure
of
the asymptomatic hyperten-
sive
subjects
to
adhere to
therapy.
ln
order
to
improve
this
deficiency, wide-
spread screening
lor
hypertension
is
necessary, and patients
must be edu-
cated to continue treatment once
and
effective regimen required
for a
particu-
lar patient has been
identified.
Side
etfect ol treatment
must
be
minimized
or counteracted
in
order to obtain the
patients' continued
cooperation.
ln ad-
dition, general measures including
re-
lief
of stress, control of diet, regular ex-
ercise etc. should
be
employed
in
the
treatment of hypertension.
