Fever, Sore Throat & Neck Swelling: Be Aware of Thyroiditis
As the thyroid is an “H-shaped” gland in the front of the lower neck, subacute thyroiditis usually presents as neck pain, low-grade
fever and palpitations (excessive heart beats), etc., others may present as unexplained weight loss and menstruation disturbance.
The infective agent is a virus; after infecting the thyroid gland, the initial phase symptoms is usually due to destruction of the thyroid
gland cells with subsequent release of thyroid hormone into the blood causing palpitation, tremor of hands, excessive sweating, and
menstruation disorders, etc. Thus, the blood T3, T4 thyroid hormone levels are elevated but the TSH (thyroid stimulating hormone),
low. Later on, there is exhaustion of the thyroid gland and production of thyroid hormone may be deficient and the patient may then
have cold intolerance, weakness, sleepiness, constipation, etc., the blood T3, T4 levels would then become low but, TSH, high. Finally,
a phase of recovery ensues, even though most such thyroid glands recover, a minority of glands may never recover: theses patients
would thus be required to take thyroid hormone for the rest of their lives. For best management, combination of Western and traditional
Chinese medicine is best.
