MANAGEMENT
OF
CONCOMITANT
HYPERTENSION AND
'HYPERURICEMIA'
Hypertension
or
high
blood
pressure
is
closely
related
to
"hyperu-
ricemia"
or
high blood uric
acid
levels.
Hyperuricemia causes gouty arthritis
or
"tophi" (uric acid crystals underneath
the
skin).
ln fact,
25"/"
of hypertensives
already have hyperuricemia; the worse
is, after diuretics (agents
promoting
urine production),
as
much as 40-50%
have hyperuricemia. Causation of high
blood
uric acid levels is
multi{actorial.
A minority source of uric acid
in the
human
body
is
from nucleic
acids
contained in certain food
items; the
majority
is
derived from
body
metabolism, especially
that
of purines.
213
ol
lhe
uric acid in the body
is
excreted through the kidneys.
Hyper-
tension (and alcohol) reduces uric acid
kidney excretion
and
increases
production.
Even
with blood uric
acid
levels
of
>300 micro-mol/L, no obvious
clinical
signs
or
symptoms occur.
Untreated, arthritic pain or even joint
deformity may occur; "tophus"
of
uric
acid crystals
begins
to appear
under-
neath
the skin; kidney stones or
even
kidney failure would eventually ensue.
All hypertensives should consult their
doctors
for
appropriate monitoring of
blood pressure and
uric
acid
levels as
well
as
the
treatment
indicated.
Self-medication
is
most
imprudent.
Hyperuricemia treatment
is enhanced
by avoiding alcohol and
purine-rich
food, e.9., internal organs,
pigeons,
haddocks and soy bean products.
