ln the
past, the treatment
ol
diabetes was mainly based on
the
fasting blood sugar level. However,
it
was
gradually
realised
that
even the
afler-meal blood sugar level is
also
important and, in difficult cases,
both
blood
sugar levels
before
and
after
food
have
to
be
taken into
consideration
when various
programmes
of
diet
control, physical exercises and other
health education are being undertaken
or
considered. Moreover, even the choice
of medication, adjusting the dose
and
deciding
on
the effective combination
of more than one anti-diabetic drugs
to
be
prescribed
may
also
depend
on
both
of
these blood sugar
level
determinations. ln addition, the
blood
level
of "HbA1c"
is
very useful, as
it
rellects the
blood
sugar
level
over
weeks rather than just a "spot" check if
the
conventional
test of
blood sugar
level is
employed. The test
for
the blood
HbAlc
level has also been employed
in
a
few recent large-scale anti-diabetic
studies, including the
UKPDS
done
in
Britain and the results
have helped
to
shed
a
lot
of
light on
the
detail guiding
principles
of
anti-diabetic drug prescrip-
tion
as
well
as
the
management of
diabetes in general.
