WHICH
ONCOLOGY
PATIENTS ARE NOT
SUITABLE FOR SURGERY?
A
vast majority of oncology
patients benefit from surgery,
for
prevention, diagnosis, staging,
cure, palliation, reconstruction or
rehabilitation.
As most
localized
cancers are operable,
loco-re-
gionalextension
and staging pa-
rameters
are
documented.
Timely excision of pre-malignant
lesions
prevents
cancer
formation. Surgical results
may
be (1) cure
(2)
failure to
control
the cancer and death of patient
(3)
cancer
recurs
after apparent
control. Certain cancer patients
are unsuitable for surgery,
e.9.,
(1) non-solid cancers, e.9.,
leu-
kemia (blood cancer); systemic
involvement,
e.9.,
of all
bone
marrows
(multiple
myeloma)
orof
several
lymph nodes
groups
(malignant lymphoma) (2)
ad-
vanced cancer, poor
general
condition; anemia, dehydration,
nutrition or
metabolic disturbance
(3)
severe
heart
or
liver
or
kid-
ney
or
lung
diseases or
high fe-
ver or
has contracted
severe
infection(s)
(4)
wide spread can-
cer
(5) cancer
sites
not easily
amenable
to
surgery,
e.g.,
nasopharynx,
upper
esophagus
(swallowing
tube),
base
of
tongue,
etc. (6)
cancers
that
spread very early on,
e.9.,
small-
cell
lung
cancer
(7) cancers
that
infiltrates
diffusely, e.9.,
cancer of
the
pancreas.
