PREVENTION
AND
TREATMENT OF PRIMARY
PULMONARY CARCINOMA
Lung
cancer, or bronchial carci-
noma of
the
lung,
is
the
most common
form of malignancy
in
the
male, reach-
ing
peak accurance after
1983 in
China.
Extensive studies
have
confirmed the
relationship between
lung
cancer
and
heavy
cigarette
smoking.
lt
has been
demonstrated that
the
more cigarettes
a
person smokes, the greater
is
his risk
of
developing cancer
in
the
lung.
lf a
person stops smoking, he is less likely
to develop lung cancer than those who
continues
to
smoke.
However, other
factors also
have been implicated.
Thus,
stringent control of source of
air
pollution
is
important to prevent the de-
velopment
of
neoplasm
of the
lung.
Considerable
attention has
been di-
rected to
the
potential role of air pollu-
tion, exposure to ionizing radiation and
numerous occupational hazards.
The clinical presentation of carci-
noma of
the
lung
depends
upon many
variables including cell type, site of ori-
gin, etc. in
many instances a patient
may
be entirely
asymptomatic.
The
early mode of presentations, such as a
mild
cough
or
a change
in
the
pattern
of
a chronic cigarette cough,
fever,
chills, sputum production, a localized
wheeze, or hemoptosis and etc.
have
special clinical
signilicance
because a
reasonable expectation of cure can be
anticipated if an early diagnosis was
made and an
appropriate treatment was
given.
ln most instances,
the
initial di-
agnostic approach
is
roentgenographic.
Other techniques such
as CT,
fiberopfic
bronchoscopy
may be also suggested
in selected patients.
The cunently preferred therapeu-
tic
approach is surgical
resection of
the
tumor.
Lobectomy rather than
pneumonectomy is the operation
of
choice,
provided
the
lesion can be ex-
cised completely by
lobectomy. Other
therapeutic approaches may be
used
singly,
or
in
concert with operative pro-
cedures.
