The Ferocious Favism
Once, several hours after a 4-year-old boy had broad beans, he had chills, fever, vomiting, tummy ache, weakness, tea-colored urine,
anaemia, a yellowish tinge in the white of the eyes and an enlarged spleen: due to haemolytic jaundice. His red blood cells were
deficient in the glucose-6-phosphate dehydrogenase (G-6-PD) enzyme: favism – the red blood cells rupture in contact with an oxidizing
agent in broad beans. Severe haemolysis may even be fatal. Favism is an X-linked recessive hereditary disease with the abnormal gene
on the X chromosome. Boys are affected but girls are usually carriers as males have only 1 X chromosome and are would then express
the disease clinically; but females have 2 X chromosomes will have a much less chance of having both X chromosomes being affected;
thus, the male:female ratio is 7-14:1. Favism is more common in South East Asia: 4.4% of boys have it in Hong Kong – haemolysis
is precipitated by having broad beans, aspirin, anti-malarials, sulphonamides; some traditional Chinese herbs and even any serious
illness. Parents must take note carefully; if any doubt, send the kid to a hospital soonest!
