Around
1/3 of
females
have
urinary incontinence;

of
them
are afraid to come for treatment.
After
the
menopause,
with
the
thinning out and loss
of
relative
resistance
to
urine
flow,
inconti-
nence is common
-
small-dose
hormone
replacement
may
benefit;
middle age
women
also
have
weakness
of
the
pelvic
floor
due
to
deliveries.
True
inconti-
nence
is
due
to infection, stones,
tumors
or
tuberculosis,
etc.
"Overflow incontinence"
is
due to
stretching and narrowing
of
the
urinary
tract, e.9., in
prostatic
hypertrophy causing difficulty
in
voiding and
over-distension
causes overflow. "Stress
incon-
tinence" is
due
to stronger
ab-
dominal pressure
than
the
sphincter can hold,
e.9.,
on
coughing. "Urge incontinence"
is
due
to
a
temporary irritable
blad-
der
arising
from
infection. Worst
cases
need
catheterization,
es-
pecially when co-existing
with
problems
of
bed
sores
(due to
constant pressure
on the
sacrum
and
worsened
by incontinence);
for males,
a
"Paul's tube" over the
penis usually suffice. Never
re-
strict water,
otheruvise
infections,
and, in
turn,
incontinence,
gets
worse.
Above
all, do
daily
pelvic
floor exercises
5
times
a
day
(10
contractions of 5-10 sec).
